I think you covered most of the points which helps you improving your language
to improve your reading learn new words and read a lot
magazines, newspapers, stories etc
to improve your listening watch movies and listen to anything in english
writing you can practise it always
one effictive way is writing your diaries
speaking just practise it whereever you go
I improved my Englisg through english course and living with english-speaking family
